It is not a clinical intake form and cannot confirm an appointment. For help with an urgent booking, call reception or use the online booking link.
How the enquiry is handled
When the website accepts an enquiry, it is stored in the clinic website’s restricted administration area so the clinic owner can retrieve it. The website confirmation means the enquiry was accepted by the server; it is not a confirmation that an email has been delivered or that an appointment has been made. This website does not currently send a clinical intake form automatically.
